Depends on what format. But usually its the title of the paper about half way down the page with your name under it. Then hit return again and add your class and under that add your teacher's name. Make sure it's double spaced. If your teacher isn't a stickler this should be fine, but you might want to check if they want the page number in the upper right hand corner.
