There have been a few cases in which the generals got promoted to the rank so early in age that they went further up becoming "Generals of the armies". This rank was proposed a six star rank but it was never practiced so the generals never wore six stars. The few cases that occurred in the 40's can be viewed on the following link.

John J Pershing was promoted to general of the armies and authorized six stars, although he never wore them. A US Postal Service stamp with Pershing wearing the six stars was issued. In the 1970's George Washington was posthumously promoted to General of the Armies.
