I recommend you use Google and do a search for a template. In my experience it does not need to be perfect to the letter, nor must it be in rigid format. Generally if you get your point across you are doing fine, outside of court rules for time limits and copies. All court rules, no matter where you are, are available to you via the internet OR the law library in the court house.
