Disease, Starvation, Abuse and being thrown overboard. Although the last one would have most likely been reserved for cases where they were too sick to be of "use" or as an example to the others. They did not want to make a trip for nothing so they weren't throwing them over left and right.
I recommend the movie Amistad. Sure it is a Hollywood movie, but not being there I can only imagine this is a pretty accurate visualization of at least one crossing.
